Placement and Support

The Placement & Support Services Department offers placement services to residents of Northwest Indiana. All placement services have achieved top level CARF accreditation.

TRANSITION SERVICES

The purpose of Transition Services is to help facilitate a successful progression from school to adult life. Transition Services staff attend student annual case reviews at the request of families or school staff.

Transition Services staff will provide families with information as to how to access other adult services. We present Arc BRIDGES program options at schools throughout Lake County.

The Transition Services staff work closely with schools, residential providers, and Medicaid Waiver case managers to ensure the most appropriate services are extended to each individual.

SUPPORTED EMPLOYMENT

Supported Employment is the main division of Placement and Support Services Department. We have a trained and qualified staff of 23 individuals whose only purpose is to place our clients in competitive employment.

At this time, we have about 200 individuals working in the community in competitive employment. The positions found for our clients are as diverse in nature as each individual. Careful attention is paid to individual preferences of employment, abilities, hours available to work, transportation concerns, and any other employment-related issues. Our current retention rate is over 76%.

Our staff consists of Job Developers, Job Coaches, Follow-Along Coaches, Employment Trainer, Coordinator of Follow Along, Coordinator of Job Development, and Employment Services Director. The Supported Employment staff assists employers by recruiting and selecting the right employee for the position. We design and implement proven and effective on-the-job techniques to accelerate skill development.

The Supported Employment staff is always available for technical assistance and support throughout the employment relationship. The Supported Employment staff is trained and qualified to provide job training, job coaching, and continuous support.

JOB CLUB

Job Club is a club which is open to all individuals in Arc BRIDGES' Supported Employment program. Job Club is held a minimum of 9 times a year and it is an opportunity for our clients to share work-related successes and/or problems. Our clients are trained to use problem-solving techniques and tools as each problem is identified.  Job Club outings are also held and this is an opportunity for all workers to have fun together in an organized activity.

COMMUNITY CONNECTIONS

The Community Connections program is designed to link people with disabilities to resources and activities in their community.  The Community Connections program provides our clients opportunities that allow for full participation and inclusion in their community.

As with all programs at Arc BRIDGES, the Community Connections program is concerned with quality outcomes.  These outcomes could include:

     Partnerships. 
     Meaningful activities and volunteer opportunities.
     Recreational opportunities.
     Developing natural supports in community activities.
     More complete integration into neighborhoods and communities.
     Increased self-esteem and an improved quality of life.

IT ALL BEGINS WITH A PLAN

The Community Connections staff participates in planning sessions with you.  We want to know what you would like to do, what resources you need to complete your chosen activities, and when you want to do your activities.

COMMUNITY CONNECTIONS MISSION STATEMENT

It is the mission of the Community Connections program to link individuals to community activities to help explore and develop their personal interests.  This program is designed to enhance empowerment, community awareness, and customer satisfaction through increased community integration

CRITERIA FOR PARTICIPATION

Community Connections participant:

     Must have a Medicaid Waiver or other funding source.

     Must have CHPI hours in their plan.

     Must choose Arc BRIDGES as their Community Connections provider.
